1. You’re registering to fundraise in support of Gold Coast Hospital Foundation, helping patients and families in critical need.

2. I declare that the information contain in this authority to fundraise is true and correct.

3. You’ll carry out your fundraising activity responsibly and in a way that reflects the values of the Foundation.

4. You won’t fundraise door-to-door, via telemarketing, or in any way that could be misleading to the public.

5. You’ll ensure your activity meets any legal or permit requirements (for example, raffles or public events).

6. Any use of the Gold Coast Hospital Foundation name or logo will need to first be approved by the Foundation before it is shared publicly.

7. Funds raised will be sent to the Foundation within 14 days of completing your fundraiser.

8. The Foundation will not accept any responsibility for any costs, loss or liability that might come from your event.

It will only be used to communicate with you about your fundraising activity, provide support, and keep you updated on the impact of your efforts.
